- Required fields are marked with an asterisk (*).
- Complete all fields thoroughly to maximize your chances of finding suitable collaborators.
Step-by-Step Instructions:
- MD+ Profile Link:*
- Locate your profile by clicking your name at the top of the Slack sidebar
- Select "View full profile" in the popup
- Copy the URL and paste it into this field
- Email:*
- Enter the email address associated with your MD+ Slack account
- Use a professional email that you check regularly, as collaborators will contact you through this address
- Role in Project:*
- Select only one option that best describes your current position: Physician, Resident, or Medical Student
- PI Information:
- Enter your full name and institutional affiliation
- Follow the format example: (Dr. John Doe, University of Medicine, Dept of Internal Medicine)
- If not applicable, type "N/A"
- Project Funding
- Please select Yes/No depending if your project has funds secured for publication costs and data collection when applicable
- N/A is for projects that don’t have publication costs (blog-posts, self-education, ect.)
- Topic:*
- Select all research areas that apply to your project
- Choose multiple topics if your project is interdisciplinary
- Project Title:*
- Provide a clear, concise title that accurately reflects your research focus
- Abstract:*
- Include a brief summary of your project, its objectives, and methodology
- If not yet drafted, type "N/A" as indicated
- Status:*
- Select the current stage of your project: Brainstorming ideas/methods, Data collection, or Manuscript writing
- TLDR of what you're looking for:*
- Clearly articulate what specific assistance you need (e.g., statistical analysis, subject matter expertise, writing support)
- Be specific about the skills or contributions you're seeking from collaborators
- Barriers to Publication:
- Describe any challenges or obstacles you anticipate in publishing this research
- Include resource limitations, methodological challenges, or other concerns

- After submitting your form, pay close attention to both your email and MD+ Slack notifications
